Important Information For Our Patients

Please read the following forms carefully. If you have any questions, please do not hesitate to ask.
Print the applicable form(s) below, fill them out, and bring them with you on your visit. This will greatly expedite the check-in process.

- New Client Form Use this form if you have not been to Lakeside Animal Clinic before.

- Anesthesia Consent Form Use this form if your pet will be going under general anesthesia.

- New Pet Form Use this form if your pet has not visited Lakeside Animal Clinic before.

- Canine Vaccination Consent Form Use this form if your dog will be having any vaccinations (shots) during his or her visit.

- Feline Vaccination Consent Form Use this form if your cat will be having any vaccinations (shots) during his or her visit.

- Boarding Check-In Form Use this form if you will be leaving your pet in our care overnight.

- Drop-Off Check-In Form Use this form if you will be dropping off your pet and picking up the same day.
Privacy Policy

We do not collect or give out any information to any third-party vendors or internet businesses. No information will ever be disclosed to outside parties from us or our website hosting company.
It's that simple! And we strictly adhere to this policy. Lakeside Animal Clinic recognizes the importance of protecting the personal privacy of its clients, its patients and those individuals who simply visit our site.
Your privacy is also protected in our hospital. At no time is any information given out about the client or the pet without the consent of the owner.


Hospital Policies

Appointment Policy

To allow ample time for all patients and scheduled surgical procedures, we operate primarily by appointment. However, we gladly accept walk-ins, both emergency and non-emergency. Appointments will have priority over walk-ins. Emergency cases shall always receive top priority, which is why occasional appointment delay is inevitable. Please realize that we make a sincere attempt to see each client on time. 

Patient Arrival Policy

For your protection, and that of others, all dogs must be on a leash and properly controlled while in the waiting area or exam rooms. All cats must be presented in an appropriate cat carrier or on a leash. 

For your convenience, drop-off appointments are available. A “drop off” means you could bring your pet at the time that works best for you and leave him/her with us for a couple of hours. Usually we will ask you to drop off sometime in the morning so our doctors can examine the patient in between appointments or at the time purposely reserved for admitted patients. Once the doctor is done, he or she will give you a call to go over diagnostic plan, treatment plan, and/or discharge instructions. 

For the safety of all animals in our care, we require that all vaccinations be up to date. Even though we make every effort to make our patients feel comfortable during visits, they may be a little uneasy about new people, new surroundings and other pets. This is one of the reasons we ask you to restrain your pet in the waiting area. We recommend that animals be placed on a leash or in pet carriers before entering the waiting room.
 
For safety and liability reasons, we cannot allow clients to restrain their own pets for an examination or any other service.
 
Aggressive Animal Policy

For the safety of our staff, we reserve the right to refuse service to a dog or cat that is showing signs of aggression. However, we will make every attempt to thoroughly examine every pet. If your pet requires a muzzle, we ask that you muzzle your pet prior to your arrival at our facility.
 
Payment Policy

We require full payment at the time that services are rendered.For your convenience, we accept Visa, MasterCard, Discover, American Express, cash and personal checks. We also offer a payment plan called CareCredit. Click here to apply for instant approval. Due to a recent increase in hot checks, we can no longer accept personal checks from new clients.
