At Lakeside Animal Clinic, we believe that the pet owner should be an integral part of the decision-making process when it comes to the health and well-being of their dog or cat. Therefore, it is important that the pet owner be as informed as possible when making important medical decisions. Open lines of communication between the clinic and the pet owner are critical when it comes to the care of your four-legged family members.

We perform a wide variety of medical procedures in our clinic, ranging from routine preventive care to life-saving surgical procedures...and everything in between. We want you to be as informed as possible before consenting to the procedures and treatments we recommend for your pet. The following forms are designed to give you as much information as possible, including possible risks, concerning any procedures we perform.

We understand that often you are in a rush when dropping off your pet at the clinic. You may not have the time required to fully read the consent forms you are signing. For that reason, we have posted them on our website so that you can read them ahead of time, and ask any questions that may arise before you drop your pet off.

Boarding/ Drop-Off/ General Consent Form
Required for all pets dropped off at the clinic when the owner leaves the premises

General Anesthesia Consent Form
Required for all pets undergoing procedures requiring sedation or general anesthesia

Canine Vaccination Consent Form
Required for all dogs receiving vaccinations

Feline Vaccination Consent Form
Required for all cats receiving vaccinations
